(7) Long description of what was stolen including visual details of the instruments (stickers, dings, modifications) and any personal story to instrument (sentimental value, inherited, etc.)

(8) Reward (IMPORTANT - You need to include if there is a reward and the amount. If there is no reward you need to specify that.)

(9) Pictures of stolen items (Attach the pics to your email. We prefer the pics to be JPG format, less than 100K each in size.)
